Please include:
- Your full name
- Employee ID or last 4 digits of SSN
- Current mailing address
- Current email address
- Phone number
- Do you want to receive your copy by mail or email?
2. Request by Phone
Call the office at (515) 224-6394 between 8:00 AM and 5:00 PM, Monday–Friday.
Forms may be sent electronically or by mail depending on your preference.
